Memorial Donation

Honoring a loss

If you would like to make a donation in memory of an individual, you can send us a check or use the online form below. Please include the name of the person you are honoring. You will receive a thank-you letter in the mail from COCA with acknowledgment of your donation.

In addition, if you would like to have a letter sent to the family or another recipient, please include the name of the deceased and the mailing address where you would like the letter sent.  COCA will send an acknowledgment letter in the mail to that address, with notification of your memorial donation.

To Send a check

• Make check payable to Colorado Ovarian Cancer Alliance
• Include a note with the name of the deceased and a mailing address if you would like a letter sent to the family.
• Mail to:

• You will receive a thank-you letter in the mail from COCA with acknowledgment of your donation.
• When provided, COCA will send an acknowledgment letter in the mail to the family, with notification of your memorial donation.

To make a Memorial Donation online:

• Include a note in the "Memorial Donation Info" line of the online transaction, with the name of the deceased and a mailing address if you would like a letter sent to the family.
  
• COCA will send an acknowledgment letter to that address, with notification of your memorial donation. 
• Once the transaction is complete, you will receive an automatic email from COCA acknowledging your donation for tax purposes. You will also receive a thank-you letter in the mail from COCA with acknowledgment of your donation.

• Step 1 - Fill out the online form below. Click the "Next" button at the bottom. This will take you to our Paypal page.

• Step 2 - Use Paypal to make your donation. To pay by credit card, click on the "Continue" link on the lower left, by the card logos . Or, you can login and use your Paypal account.

• When you make a donation, you become a FRIEND OF COCA and your name is added to our annual Friends of COCA list. Check "No Listing" if you prefer your name not be added to our published list.
